how Do We Support Primary Health networks (PHns)?
Our team understands the important role that PHNs play at a regional level to increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time
As a mission-based health and social services consultancy, Beacon Strategies is committed to partnering with PHNs to better plan, design, implement, evaluate and communicate their work in delivering health outcomes for local communities.
Over several years, we have worked with PHNs across Australia to guide and support their work at various stages in the commissioning cycle to improve health outcomes, tackle health disparities, address service gaps and support the primary care workforce across areas such as mental health and suicide prevention, aged care, palliative care, alcohol and other drugs, and population health.
Health planning: understanding and articulating the health and social needs of the communities a PHN serves through comprehensive health needs assessment processes. We support PHNs to develop sector-endorsed planning documents and implementation resources that effectively respond to identified needs.
Service design: developing service models that are evidence-based, responsive to local need and informed by industry-leading knowledge and techniques. Our service design offering supports PHNs to translate regional and strategic plans into detailed service models, ready for investment.
Project management: supporting PHNs with time-limited, intensive and industry-specific implementation support to ensure that vital commissioning activities are adequately resourced. Our industry-specific experience working in the PHN environment ensures our team is ready to support our PHN partners and offer genuine value.
Evaluation and program review: undertaking qualitative and quantitative outcome evaluations, program reviews and process improvement exercises, as well as developing meaningful reporting products to influence stakeholders, inform funding decisions and prompt service redesign.
Communication and change management: effectively connecting and communicating with a PHN’s diverse stakeholders, including facilitating stakeholder engagement processes and planning for successful change management.
Training and facilitation: working with internal PHN teams to identify strategic problems, explore solutions, build capability and plan for change through expert facilitation.
