The purpose of delivering the Palliative Care and Older People (with a focus on residential aged care facilities and after hours) Commissioning Project is to provide evidence-based information and undertake stakeholder engagement to inform the design and delivery of an agreed regionalised approach to GCPHN investment in palliative care and an after-hours response in relation to aged care.
Progress toward the overall goal will be achieved through development of a clear, organised and connected suicide prevention clinical care pathway across and within the Brisbane North PHN region, which has high utility across sectors and enhances the care and support for people at risk of suicide and for people who require aftercare/follow-up after a suicide attempt, or who are bereaved.
United Synergies has a significant body of information that has been developed over past years in relation to Standby and has engaged Beacon Strategies to undertake a planning process for successful engagement with prospective funders into the future. Following the project, United Synergies will be better informed to undertake targeted engagement with PHNs and other prospective funders across Australia, based on alignment between the value proposition of StandBy service and the needs and priorities of funders.
Beacon Strategies will deliver a comprehensive regional service profile outlining the current capacity and projected demand for services within the Brisbane South region, along with a strategic lens document outlining key considerations for future planning within the region, with a particular focus on vulnerable consumers such as Culturally and Linguistically Diverse (CALD) populations.
The delivery of a training package for Ipswich City Council Community Development staff was geared toward improving the departmental approach to identifying and articulating community need across a range of issues, designing and documenting effective approaches to meet identified needs and establishing and implementing evaluation frameworks to gauge the effectiveness of a range of initiatives over time.

The purpose of the scoping exercise was to explore the feasibility of fully implementing co-commissioning processes (co-planning, co-design and potentially co-funding) for MHAOD services within QLD within the next five years, through describing the existing co-commissioning landscape at a policy level, summarising consultation undertaken with Queensland PHN representatives, presenting implementation guidance for the QLD PHN CEOs working group, mapping a potential way forward for co-commissioning activities for QLD PHNs and being a key point of reference for any related project activity in the future.
