how Do We Support Primary Health networks (PHns)?

Our team understands the important role that PHNs play at a regional level to increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time

As a mission-based health and social services consultancy, Beacon Strategies is committed to partnering with PHNs to better plan, design, implement, evaluate and communicate their work in delivering health outcomes for local communities.

Over several years, we have worked with PHNs across Australia to guide and support their work at various stages in the commissioning cycle to improve health outcomes, tackle health disparities, address service gaps and support the primary care workforce across areas such as mental health and suicide prevention, aged care, palliative care, alcohol and other drugs, and population health.

Our PHN Health Planning and service design Work

Our health planning and service design work enables PHNs to make informed investment decisions to meet the needs of their region. We support PHNs to commission services that are evidence-based, responsive to community needs and endorsed by the community and sector. 

We have experience in delivering a range of products including population health needs assessments, health and service plans, regional plans, commissioning plans, market approach documentation and a variety of other strategy and program documents. Our larger pieces of work often include a combination of the services listed below.

Community conversations: community involvement is key in the development of any PHN funded service or initiative. We engage with the community through focus groups, conversations and surveys to ensure that the voice of the community is built into regional planning processes and service models.

Stakeholder consultation: we have demonstrated capability in leveraging the knowledge of PHN portfolio and commissioning focussed staff, local topic experts and service providers when developing regional health and service plans. We engage with expert stakeholders to unlock years of experience, avoid pitfalls, understand the local context and generate buy-in with your key stakeholders.

Service mapping: Understanding the local market enables PHNs to understand the potential for future service delivery in addition to service duplications and gaps. Maintaining a broad spread of services is vital to achieving a well balanced system. We support PHNs to understand the service provider market through service mapping and market analysis.

Evidence review: establishing a base of evidence makes the process of specifying how programs and services are delivered a lot easier. Having a poor understanding of the key elements of investing in a particular service model causes ineffective service delivery and misguided service expectations. We have access to academic databases and are highly skilled at developing concise evidence reviews to inform strategic priorities.

Population level data analysis: understanding the need for a program or service is critical for commissioning services. Defining population level need shapes how strategies are developed including who should be targeted as a priority and where the service should be located. We provide an expert level of data analysis to determine the overall need and targeting for future activity.

Our PHN Evaluation Services

We work collaboratively with PHNs to evaluate and/or review the impact of existing programs and services to inform commissioning decisions. We support PHNs to:

  • Develop evaluation plans to determine evaluation scope and guide evaluation activities

  • Evaluate client and program level outcomes

  • Identify factors that underpin successful delivery of the service

  • Report back to PHN decision makers and other relevant stakeholders on findings and considerations relating to future delivery of the service model

A typical evaluation project on the behalf of a PHN would encompass a number of evaluation techniques, including analysis of quantitative service data, collection of additional qualitative data (surveys, interviews and focus groups), evidence review and horizon scanning (to position the findings in appropriate context) and development of reporting products.
