Commissioning early intervention initiatives for Brisbane North PHN

Background

The purpose of the project is to undertake intensive desktop research to support Brisbane North PHN to consolidate their knowledge of the needs of the region and prioritise potential commissioning responses to support the identification of early intervention initiatives that promote healthy ageing, slow decline, and support the ongoing management of chronic conditions for older adults.

Additionally, the project has a specific focus on older adults at risk of poor health outcomes such as Aboriginal and Torres Strait Islander peoples, people from Culturally and Linguistically Diverse (CALD) backgrounds, people who identify with Lesbian, Gay, Bisexual, Transgender, Intersex and Queer (LGBTIQ) communities, homeless, or at risk of being homeless, and those socially isolated.

Approach

Our project team will undertake the project via a series of phases that will be completed and delivered as a long for report for the consideration of Brisbane North PHN. Project phases include;

  • Phase 1: project establishment — develop key project management documentation, enabling visibility of project approach and scheduled activities. 

  • Phase 2: desktop research (needs) — review existing Brisbane North PHN documentation and scan available literature relating to the needs of older adults aligned with the target groups of the project.

  • Phase 3: data analysis (needs) — analyse population demographics, health status and service utilisation data regarding older adults to quantify the scale and scope of future services. 

  • Phase 4: desktop research (models) — we will scan reputable information sources to identify a broad range of delivery models that will be used to shape future phases of the commissioning cycle.

  • Phase 5: service mapping — we will map all existing relevant programs and services delivered within the Brisbane North PHN region to identify gaps, avoid duplication, and encourage providers to leverage existing infrastructure.

  • Phase 6: reporting and recommendations — we will compile the findings of each of the project phases and submit a report to Brisbane North PHN regarding implications and recommendations for next steps.
