Service Mapping ‘Greater Choices for At Home Palliative Care’ for Western QLD PHN

Background

The purpose of this project is to collaboratively design a service mapping tool in partnership with Western Queensland PHN.

Approach

Our approach to collaboratively developing a service mapping tool with WQPHN is to:

  • rapidly analyse situation

  • define data indicators

  • develop a conversation guide

  • design a data collection tool

  • brief PHN team on data collection

To ensure that the service mapping tool meets the needs of Western QLD PHN, we will rapidly analyse the situation, context and drivers. We will request Western QLD PHN turn over relevant documents to support this initial step as well as engage with key Western QLD PHN staff to understand project requirements.

We will facilitate a series of meetings to develop an agreed set of indicators to guide a consistent approach to data collection. Additionally, we’ll develop a conversation guide to ensure clarity of messaging and consistency during the data collection phase.

In collaboration with Western QLD PHN, we will develop a data collection tool to ensure consistent data capture. The data collection tool may be a webform, spreadsheet or document, but will aim to collect clean and consistent data.

Lastly, if Western QLD PHN opt to collect much of the data themselves, we will prepare guidance and brief the PHN team on how to administer the service mapping data collection tool.


Our team understands the important role that PHNs play at a regional level to increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time.
