Situational Analysis and draft strategy development for Northern Queensland PHNs Chronic Conditions Strategy

Background

NQPHN is developing a five-year Chronic Conditions Strategy to support the commissioning of services that improve primary care prevention, early intervention, and management of chronic conditions where there is the greatest need. In addition to informing commissioning priorities, the strategy may also support and inform system integration and innovation, workforce development and primary care engagement priorities in relation to chronic condition primary care service delivery.

To inform the development of a Chronic Conditions Strategy, Northern Queensland Primary Health Network (NQPHN) engaged Beacon Strategies to undertake a situational analysis of the strategic environment, operational environment, population health needs and evidence base relating to chronic conditions relevant to the NQ region.

Approach

The objectives of the project are to:

  • undertake a targeted desktop review of a range of source documents and evidence to develop a situational analysis report

  • develop a draft strategy document that outlines NQPHN’s role in contributing to better outcomes for people experiencing chronic conditions in the region

  • support NQPHN to transition into the subsequent stage(s) of the strategy development process through targeted advice.

Our team understands the important role that PHNs play at a regional level to increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time.
